Question about gauges in the medical field

This topic is brought up often of gauges being unprofessional, and ”you’ll never get a good paying job with those” is something i hear very often. I know this is false, but i’m interested in becoming a surgeon, and i have 19mm gauges. of course i cant wear my tunnels to that job at all, but should i size down still so my bare earlobes aren’t too noticeably stretched? this might be a dumb question considering it would be ridiculous for bare earlobes to be an issue, but people are dumb, you never know..
